Can I tour your facility?

We welcome you and your family to tour our facility any time. Please call ahead to let us know you are coming. If you want to visit the clinic during patient care hours plan on coming between 8:30-12:30 and 1:30-4:30.

Do you have a waiting list?

We often have a wait list, but we will look at your situation individually. If you have had a recent stroke or brain surgery we will make special arrangements.

How do I register?

Have your doctor fax a prescription and we will contact you. Our fax number is (602) 393-0523

What happens during my first visit?

During your first visit you can expect the following:

  • Arrive at your appointment 15 minutes early to complete your paperwork or you can download it from our website - see forms link - and bring it with you.
  • If you have a prescription for therapy bring it with you.
  • We will copy your insurance card.
  • You will be seen for the initial evaluation by a therapist.
What do I need to bring with me?

Your eager and smiling self, friends and/or family if you would like, an identification card, insurance cards and any specialty equipment you use (splints, braces, canes, walkers, etc.) Please arrive 15 minutes early if you don't have your paperwork already filled out prior to your visit.

How should I dress?
You should wear comfortable clothes and shoes that you would normally wear for exercise.

How long will each treatment last?

This will be discussed and decided by you, your family, and your therapist at your initial evaluation.

Who pays for the treatment?

In most cases, health insurance will cover your treatment. We will pre-authorize your insurance coverage before you start therapy. Extra services and some equipment may not be covered by insurance.

What types of insurances does SWAN accept?

We accept Medicare, Medicaid, Blue Cross Blue Shield, United Health Care, Aetna, Cigna, Humana, VA and Tricare. Call our office for other insurances or to discuss insurance benefits (602) 393-0520.

Who will see me?

You will be evaluated by one of our licensed and highly trained therapists and a therapist will also treat you during subsequent visits. This team consists of Physical Therapists, Occupational Therapists and Speech Therapists.

Can I make improvements in function, even if it has been years after my stroke?

Yes. New research in the appropriate type of therapy and in new technologies have proven that functional gains can be made, even years after injury. HOWEVER, it takes a lot of work on your part as well as ours.

Can I bring my wife/husband/caregiver?

Absolutely. We encourage your caregiver to be part of therapy, and in many cases he/she can be with you during therapy. We may ask your caregiver to wait in the waiting room at times.

How many visits will I need?

This is highly variable. Again, this will be discussed with you at your initial evaluation.

Is therapy painful?

We try hard to make therapy pain free as well as fun.

What is your cancellation policy?

SWAN Rehab schedules a high ratio of therapist to patient. Therefore, it is imperative that if you are unable to make your scheduled appointment you provide a 48 hours notice. If we are not open you can leave a message. Cancellations made without a 24 hour notice will result in a $50.00 per hour fee. If more than 3 appointments are cancelled or missed within a treatment series all future appointments will be cancelled unless special arrangements are made with SWAN administration.
