Pam Rivero, PT, CEEAA, cert-LSVT
Pam has a passion for people and believes that every therapist has an opportunity to change lives. For more than ten years she has been helping other therapists deliver invaluable service to individuals, by putting the person at the center of what we do in rehabilitation, expecting excellent care, empathy and compassion, never giving up and keeping hope alive. She creates an enriched environment so that people can achieve optimal potential, all people feel welcome and are inspired to exceed their goals. Pam has been a Physical Therapist since 1999 and is a proud graduate of Pacific University in Oregon. What inspired her to become a Physical Therapist is the empowerment that the therapist can bring to the people they work with. By helping people help themselves become independent they feel confident, strong and empowered to meet any challenge.
Pam enjoys spending time with her husband watching their two boys play Lacrosse. When she is not working or on the field, she is snuggling with her three dogs. She also tries to get up to the Pacific Northwest in the Summer to visit her family, along with the mountains, beaches and waterfalls.
Rachel Dorsey, DPT
Outpatient Program Manager
Rachel was born and raised in Holbrook, AZ and is a true Arizonian. She completed her undergraduate studies at the University of Arizona and received her Doctoral Degree in Physical Therapy from Northern Arizona University. She has years of experience in the treatment of Neurological, Orthopedic, Cardiopulmonary, Vestibular, and Geriatric conditions in Hospital, Outpatient, Community, and School-based settings.
Rachel has been an active member of the Arizona Physical Therapy Association since a student. She is a leader in community affairs, and founded the Friends of the Holbrook Animal Shelter non-profit organization in Holbrook, AZ which received the Organization of the Year award. Rachel is a believer in the SWAN rehab philosophy, particularly in their interdisciplinary approach. She is energized by her role as the outpatient clinical team leader and the opportunity to bring her passion for patient care to SWAN.
Comprehensive Center for Vocational Rehabilitation Program Manager
Sarah is a graduate of Arizona State University with her Bachelor's degree in Recreation Management with an emphasis in Therapeutic Recreation. She has worked in outpatient neurological rehabilitation and physical medical rehabilitation for the past 19 years in various positions including: Recreational Therapist, Director of Recreation Therapy and Extended Rehab, Community Reentry Specialist and Vocational Rehab Specialist.
Sarah's accomplishments include being awarded with Gentiva Extra Mile Award in 2008, ResCare Sue Scheuner Award for Clinical Excellent in 2011 and the 2013-2014 Volunteer of the Year Award through the City of Mesa and Arizona Disabled Sports.
Michelle Whalen, PT, DPT
Doctor of Physical Therapy
Michelle is originally from Orange County, California. She received her Doctorate of Physical Therapy degree from Mount Saint Mary’s University in Los Angeles, and received her Bachelor's degree in Biology-Physiology and Minor in Chemistry from California State University of Long Beach. Michelle loves working with the Neurological population and assisting patients with regaining strength, function, and independence. Michelle had played soccer for 21 years and now spends her spare time hiking with her husband Michael and their dog, Maple. Michelle also enjoys working with ceramics and polymer clay as hobbies.
Kimberly Chesney, PT, DPT
Doctor of Physical Therapy
Kimberly earned her Doctorate of Physical Therapy from Northern Arizona University in 2016. An Arizona native, Kimberly grew up in Prescott and moved to Phoenix to start her career at SWAN Rehab. She is dedicated to providing quality and compassionate care for those living with neurologic conditions. In her free time, Kimberly trains and competes with her horse in the sport of three-day eventing, and also enjoys hiking, camping and cooking.
Jessica Fairbairn, PT, DPT
Doctor of Physical Therapy
Jessica is originally from the suburbs of Chicago. She received her Doctorate in Physical Therapy from A.T. Still University and received her Bachelor's degree in Athletic Training at the University of Illinois Urbana-Champaign. She has experience in outpatient therapy and is excited to be a part of the SWAN team. She is dedicated to providing quality and patient-centered care. She is on the board of the Arizona Physical Therapy Early Professional Special Interest Group and is an adjunct faculty for A.T. Still University assisting in Clinical Anatomy for future physical therapists. In her free time, Jessica enjoys hiking with her husband and dog as well as cooking. She also enjoys volunteering with adaptive sports.
Amanda Wolf, PT
Amanda is originally from Wisconsin. She went to college at University of Wisconsin – La Crosse and received her doctorate at Midwestern University. She loves working with people with neurological disorders and helping them restore their function. During her time off, she enjoys cooking, hiking and cheering on the Green Bay Packers.
Kate Hirsch, PTA
Kate earned her Associate in Applied Science from Gateway Community College Physical Therapist Assisting Program. Kate grew up in Phoenix, Arizona and received her Bachelor of Arts in Psychology at the University of Arizona in 2007. Kate decided to pursue a career in physical therapy and discovered her passion for working with the neurological population. She is excited to be a part of the SWAN team and enjoys hiking, yoga, and traveling with friends and family.
Physical Therapist Assistant
Logan completed his undergrad at University of Montana (Go Griz!). He will be applying for PT school this year and hopefully staying here in Arizona. He is a twin, a native Phoenician, enjoys college football, hiking and making coffee. He looks forward to applying the knowledge and patient care that he has learned here at SWAN.
Vicki is a native Phoenician who has devoted her life to helping others in her role as a rehabilitation technician. She worked at St. Luke’s for 13 years in their therapy department until they closed. When she is not at work spreading hope and helping other’s she loves spending time with her family and two dogs (Max and Zoe) hiking, camping and anything outdoors.
Kristen was born and raised in Peoria, AZ. She received her Bachelor's in Psychology from Arizona State University in 2012 and is waiting to start the process of applying to Physical Therapy schools in the summer. Kristen loves spending time with her family and friends in addition, to helping others. Kristen also enjoys rooting for her two favorite hockey teams, the Detroit Red Wings and the Phoenix Coyotes, working out, country music, and food. She feel very blessed to work with the SWAN team members and wishes to provide as much happiness and humor to others as life granted her.
Breanna Dennis, OTR/L
Breanna received her Master's degree in Occupational Therapy from A.T. Still University in Mesa, AZ and a Bachelor's degree in Human Health Studies at Arizona State University in Tempe, AZ. Breanna is a proud native and was born and raised in Chandler, AZ. In her spare time, Breanna enjoys traveling, camping, playing volleyball, bowling, and wrestling with her three pups. Breanna has worked with neuro patients her entire career in many different settings and is excited to be a part of the SWAN team to share her experience and passion for recovery with you.
Chardae Durden, OT
A native of New Mexico, Chardae knew she wanted to become an OT at the young age of 15. She has degrees in Biology and French, and in 2017 completed her Master's in Occupational Therapy from the University of New Mexico in Albuquerque. Chardae is fluent in French and enjoys drinking tea, reading, cooking, fitness and cuddling her dog "Iroh”.
Samantha Eagle, OT
Samantha was raised in Southern California. She completed her B.A. at Scripps College and earned her M.S. in Occupational Therapy from Washington University in St. Louis in 2018. Samantha’s graduate research focused on creating self-management fact sheets for individuals with mild stroke. Samantha played tennis most of her life and now enjoys CrossFit and running in her free time.
Sarah Colquitt, OT
Sarah was born and raised in Acworth, GA. She moved to AZ after attaining a Master’s of Science in Occupational Therapy. She has a Bachelor’s of Science in Psychology as well. Since living in AZ Sarah has enjoyed working with various populations and settings, her favorite of which is assisting those with neurological conditions. She loves a good challenge and thinking outside the box. When not working Sarah likes to play with her dog Coco, go on short trips, hike, bike, spend time with loved ones, and camp.
Darwin Johnson, COTA
Certified Occupational Therapist Assistant
Darwin graduated from the University of Arizona with a Bachelor’s degree in Psychology. From here, he went straight into OT school at Pima Medical Institute where he developed a love for neurological rehabilitation. In his free time, Darwin loves being outdoors hiking, spending time in the gym, taking daytrips or volunteering his time to various causes. He is excited to join the team and grow as a therapist.
Annette is a Speech-Language Pathologist who joined SWAN Rehab in 2012 because of her desire to provide excellent therapy for people with neurogenic communication disorders. She received her Master of Science degree in Communication Disorders from Arizona State University where she gained valuable experience working in the Aging and Adult Language Disorders research lab. She is our ALS Speech Therapist. Annette is a member of the American Speech-Language-Hearing Association, the national professional credentialing association. At the state level, Annette is a member of the Medical Special Committee for the Arizona Speech-Language-Hearing Association. She is proud to be a member of the SWAN team.
Janet was born and raised in Wisconsin. She completed her undergraduate degree in Communication Disorders at the University of Wisconsin-Eau Claire and her Master's degree in Speech-Language Pathology at Arizona State University. Her primary focus throughout her career has been the rehabilitation of individuals with neurological disorders. She considers it both a pleasure and a privilege to work with this population. Janet is excited to be a member of the SWAN team. She enjoys early morning walks with her yellow lab Sydney, spending time with friends, hiking, yoga and sports./p>
Administrative Assistant Comprehensive Vocational Rehabilitation
Trina is a native Phoenician and have never really ventured too far. She loves living in Central Phoenix where she can bike to dinner or the farmers market, hike some of the best trails and enjoy the closest thing to big trees and green grass while living in the desert.

Krendi is a native of Michigan and moved with her family to Arizona in 2008. She has an extensive administrative background and is our Administrative Assistant for our Comprehensive Vocational Rehabilitation. When not at work, Krendi enjoys being with her family, watching movies, and loves board games. She also loves exploring Arizona.
Tessa has newly joined SWAN Rehab as our medical receptionist. She is a native of Salt Lake City, Utah and therefore will spend all of her days in Arizona adjusting to its heat. She has a Bachelor of Arts in English from the University of Utah and enjoys freelance copyediting. She has a passion for helping people in whatever capacity she can. She loves socializing with her family and friends and delights in hiking, reading, and puppies of all shapes and sizes. She is excited to be a part of the SWAN team and to contribute to the HOPE that it brings to all who enter its doors.
